PATIENT INTAKE PROCESS

The intake process begins with the completion of the patient questionnaire. Please complete one form for each child that you would like to enroll in the practice. Once we have reviewed the form, we will contact you to initiate enrollment. Each child would be enrolled in the patient portal of the electronic medical record at that time and the membership fee would be assessed. Since Simplicity Pediatric Consulting offers longer appointment times and unlimited visits, enrollment eventually will be capped.

FEE SCHEDULE

  • $175/month for one child
  • $150/month additional for the second child
  • $125/month additional per child for the third and fourth children
  • $600/month total for a family of five or more children
