PATIENT INTAKE PROCESS

The intake process begins with the completion of the patient questionnaire. Please complete one form for each child that you would like to enroll in the practice. Once we have reviewed the form, we will contact you to initiate enrollment. Each child would be enrolled in the patient portal of the electronic medical record at that time and the annual membership fee would be assessed. Since Simplicity Pediatric Consulting offers longer appointment times and unlimited visits, enrollment eventually will be capped.

ANNUAL FEE SCHEDULE

  • $1550 for one child
  • $1300 for the second child
  • $1050 each for third and fourth children
  • $5250 total for 5 or more children
